From bottom to top:

1. Dealer
2. Floor
3. Pit
4. Shift
5. Table Games Supervisor
6. Casino Manager
7. Casino President

I think somewhere between 6 and 7 you are missing:

No, that is just 5 and 6 and 7's job.

That looks fairly accurate at most places. Floors are sometimes called "table games supervisors".

Also, 4, 5, 6, or 7 could be in charge at any time. I've even seen a "pit boss" be in charge of all table games at a small place.

Once, I had 5,6 and 7 all show up at my table at once. The best part was I didn't even get backed off: they just wanted to talk to a whale at my table!

An Executive Host has more power than you can believe.
I have used them extensively when ' competing' in the casino against other slot APs.. They r very high on the totem pole.
