Most ridiculous dealer error

I was recently making a first pass through a casino I'd never been in, checking on conditions, when a player celebrated a winning hand at a carnival game, and generated a little bit of a commotion. I walked over to see what was going on and saw that the bet was a mix of red and green totaling $200. The dealer counted out the bet, counted out $450 to pay him, and announced to the floor that he was paying $450. The floor came over, looked at the hand, looked at the bet, and informed the dealer that the bet was only $200, so the payout was $400. The dealer then proceeded to pay the $400. I read the felt to figure out why the bet was being paid 2:1. Eventually I even (discreetly) asked one of the other players if the hand was supposed to be paid even money or 2:1. Turns out the floor fixed a $250 bonus by turning it into a $200 bonus.
