some questions for Indices for KO Count

Hello, i have some questions regarding indices in KO count.

according to qfit.com the indices for KO count on 6deck DAS S17 game are that i should put Insurance on the following hands :

16vT
15vT
12v2
12v3
12v4
11vA
10vA
10vT

in my city early surrender is allowed [ YES EARLY SURRENDER - this is EU BJ with poorly 66% pen]. So the basic strategy for this game on 16vT, 15vT and 14vT is to surrender.

My questions here is what to do on 2 cards 14-15-16 vs T - surrender or surrender + insurance.

also i cannot understand why the indices say :

surrender for 14vT (includes 7,7vT) and insurance for 10-15-16 vs T. why 14 should not be with insurance when 10-15-16 are ?
