Lucky Ladies (Table C - 1 Deck) and Sweet Sixteen availability

I have been looking into all the Blackjack side bets described at the
wizardofodds.com site, to see which ones might be beatable.

Two of them in particular look like they may be beatable - "Sweet Sixteen",
and "Lucky Ladies" with 1 deck and the pay table (Table C) which pays on
any pair of queens (not just Q hearts).

Does anyone happen to know if Sweet Sixteen is still available in any land
based (countable) casinos? Or has this side bet died out?

And is "Lucky Ladies" currently available in the "Table C - 1 Deck" form?
If not, is Table A the norm, or the more generous Table B?

I see another potential problem with making an income from "Lucky Ladies"
to be the low maximum bets.
I believe I've seen it advertised with a $100 max bet, with $25 probably
more common? Are there casinos that accept even higher bets on Lucky Ladies?
Ideally I'd like to be able to bet as much as $500 a hand, given that any
player edge is likely to be quite low.

It would be great to hear from people with more experience of casinos than
myself, particularly experience of US casinos. I live in the UK and the few
UK casinos I have visited offered only the 21+3 side bet, which I don't think
is very beatable. The main Blackjack rules also tend to be very hard to beat
here in the UK.
