On what basis, and was it regarding casino's? Also do you have any references?
What's the official word on novelty IDs? I know it's illegal to use them with intent to commit fraud, but is using a fake ID to obtain a player's card considered fraud if the player is able to collect comps? Is it fraud if the player has not been previously barred from a casino? I know this was the subject of discussion recently, but I don't think there was any definitive answer.
Are there other, less risky, methods of obtaining a player's card in a different name?
Well I hold a canadian driver's license, and for an odd reason, they have last name first and first name last, so I can switch my names I guess. And as for address, I can always use a P.O Box back home, i'll try that today. The only thing I don't like about this is, they will track my play, and when they see a pattern, won't they catch on?
If you have a friend who's approximately your age, have him get a card for you. Memorize his address and his birthday.
If you don't know anyone in Vegas, you can go into a poker room, befriend someone who plays poker and who's a local; and try to get him to do it for you. This is a bit more risky, because you're basically confessing to a stranger that you're a card counter. I know someone who HAS successfully used this method in the past. What YOU get out of it is the ability to resume play in places where you have a problem. What HE gets out of it is whatever COMPS you may generate. If you can comfortably explain it THIS way to someone, and if he's smart enough he'll GO for it.

You can always surf the slot areas for lost cards. Doesn't help you with comps, but it lets you avoid trying to play unrated.
A lot of elements come into play, and I think then there is a legal ground for getting charged, because of identity theft, and or taking someone else's property.
Do you know any college students? If so, ask them where to get the fake ID. My daughter tells me the going rate where she goes to school is $150 for a high quality (w/hologram) non-scannable ID. Scanning is extra
Another possibility is ask an illegal (like the guys on the strip handing out the porn cards)

I don't plan on using this card on any form of government authority, it would be extremely stupid.
With newer technologies this is becoming very risky. I constantly try to read the pits screens and the amount of info at some casinos is scary. I've seen players pictures pop up on the screen. Also your age seems to be pretty standard as well. If you are in your 20's and you pick up a card from a 70 yo it will raise instint suspicion in the pit.
Where do they get the picture? From the scanned drivers' license we use to sign up with?
P.S: I've been playing 2 hours at every casino with best games, though it's been fun, variance is kicking my ass, down about $6000, just bizarre events, the count is high, and a 16 becomes a 21 for the dealer. Of course considering my two-year experience, it's nothing new, but still a little challenge to accept it when it happens so frequently at times
