Percentage of face-up single and double decks?

What percentage of single(ignoring the 6-5 ripoff games) and double deck games in the US are dealt face up? I know these games are face up at the El Cortez in Vegas, but I'm curious about general conditions.

Single and double deck Super Fun 21 is ALWAYS dealt face up. Ignoring those games along with 6:5 games, the percentage is probably about 1%.

Not very many. El Cortez and Belterra in Indiana are the only ones I can think of off the top of my head.

I sort of remember a DD game in Laughlin dealt face up, but no DAS. Harrahs maybe?

A very long time ago in Reno I played DD shoe game that was dealt face up. I think it was the El Dorado. Does anyone know if they still do this there?
