Why do most bet spread switch to 2 hands at the upper level? Is 2 hands of $200 better EV than 1 have of 400? If so, why not play 2 hands through entire bet spread where table minimums allow it?

Lower variance with two hands on big bets. With minimum bets you want the least amount on the felt as possible. If the minimum is $10 by playing two hands youre playing two hands at $20. Now youre playing $40 with no edge instead of $10

No, it's worse EV, because it uses up more cards for the same EV. If you spread to two hands, in order to keep the same ROR as for one hand, you have to bet approximately 75% of the one-hand wager on each of the two hands. So, if you were to bet $400 on one hand, you ought to bet two hands of $300 each. That way, your EV increases but you ROR remains the same.

Finally, this works best when there are other players at the table with you and you aren't playing alone.

I don't switch because of the heat it creates.
Its 1 hand heads up the whole time, no matter what. Even if you are getting smoked on a high TC situation, you keep the one because of what DSchles said, you want those rounds. If that penn is deep ... you are really in advantageous situation so just rip one hand as fast as possible.
When I am with someone else its 2 hands the whole time. You need to try to get more of those high cards, but sometimes the co variance will take you to the moon, take you to the grave, or create a break even situation.
