I just read the helpful analysis of the lucky ladies side bet. The payout structure used for the sim was the following:
Q hearts pair w/ dealer BJ - 1000:1
Q hearts pair - 125:1
Matched 20 - 19:1
Suited 20 - 9:1

Your best bet are the card rooms in Tukwila (20 minutes south of Seattle). There are 4 grouped together and one with a double deck (not great penetration though). Avoid the Tribal Casinos (Tulalip). The rules are not good.
